Case history:

This person states that her dentist has told her that her center right front tooth is severely compromised and will need to be extracted. From the picture that she submitted, you can see that this tooth already has a dental crown on it. The purplish color of the gum tissue surrounding this tooth suggests that all is not healthy.

Beyond this immediate problem, the remainder of her smile suffers from a generalized tooth misalignment due to tooth crowding (there is not enough jawbone space to accommodate the cumulative width of her teeth).

About our approach to this cosmetic dentistry makeover simulation.

Replace the missing tooth with either a dental implant or a dental bridge.
One of the major cruxes of this smile makeover will have to do with the dentist's ability to make the gum line associated with the new artificial tooth (the right central incisor) at the same level as her upper left center tooth (left central incisor). If a dental bridge is used to replace the tooth, this feat should be fairly easy to accomplish since the artificial tooth will simply overlap the gum tissue. If instead a dental implant is chosen, the implant will need to be positioned with precision. The ability to do so will be dependent upon the size, shape and quality of the jawbone in this region.

Changing the apparent alignment of the other teeth.
The rest of our makeover simply involves resizing and reshaping the remaining front upper teeth (by way of placing dental crowns, porcelain veneers, or possibly even just dental bonding) so to give them an apparent alignment that is more pleasing. One factor that will keep this smile makeover from having a perfectly symmetrical appearance is this person's right eyetooth (cuspid). While the dentist can change the dimensions of this tooth, they will not be able to change it's overall positioning. It would take orthodontic treatment to reposition this tooth and subsequently give it a more pleasing gum line level.
