There are a lot of issues going on with this case. The most basic and fundamental are that this person is simply not taking care of their teeth. It is easy to see a heavy accumulation of dental plaque a long the gum line of most all of the teeth. Due to this it seems that a cavity has formed at the gum line edge of the upper left central incisor. Additionally, the gum tissue closest to most all of the teeth has a dark red coloration. This is typically a sign of gum disease.
It is possible of course to make cosmetic improvements with cases such as these. Before any changes could be considered at all, however, a thorough dental cleaning and examination would have to be performed. It is quite likely that a serious gum condition exists here. And if so, clearly it must be rectified before any elective cosmetic dental work should be placed. This patient would also need to understand that the long-term outlook for any restorations that are placed will most likely be seriously compromised unless adequate oral home care is begun and carried on.
