Case history:

The cosmetic dilemma associated with this smile involves this person's central incisors. Clearly their alignment is irregular. Possibly these teeth are slightly undersized (in width) as well.

This case is a slam-dunk. By just placing dental crowns on the center two teeth the whole smile is transformed. However, we would have two things to say about treating this case.

• One big question, at least to a dentist trying to create a result with the best possible long-term outlook, is why were the center two teeth (the central incisors) crooked to start with? There seems something slightly strange about the "before" alignment of this person's teeth. There is some possibility that they have shifted as a result of being compromised by advanced periodontal disease (gum disease). If this is the case then the periodontal disease needs to be resolved first. Afterward the crowns can be placed. Otherwise the teeth will continue to shift and the gum disease will worsen.
  
  
• The other issue of concern here is that dental crowns can be used to change the apparent alignment of teeth (as we have simulated in our "after" picture) but the position of the root of the tooth is not changed. This can create a cosmetic mismatch that produces a strange tooth-to-gum relationship and appearance. With this case, some sort of very minor orthodontic treatment using a removable appliance (such as Invisalign Express ®) for a few weeks might be needed so to idealize the position of the right central incisor before the dental crowns are placed.
