Our "after" picture shows an outcome that we think might be possible to accomplish with this case. All of the changes we have illustrated are the type changes that dental crowns can typically make for teeth. The idea is to use the dental crowns to restore the worn teeth back to their original shape and to reduce the size of the upper eyeteeth so overall this person's smile takes on a more classical appearance.
Probably the most important factor associated with a dentist being able to accomplish the changes we've illustrated will have to do with this person's bite. If, when this person's back teeth are fully together, their front teeth overlap then there is a possibility that the solution we've illustrated might be possible. If instead the front teeth meet edge to edge then the treating dentist may or may not be able to make significant shape changes with the teeth.
Either way the longevity of this cosmetic makeover will be compromised if this person continues to clench and grind their teeth. The treating dentist must make it clear to this patient that their bruxism habit must be controlled. Dentists often make plastic appliances (night guards) which help to protect teeth from this activity when a person sleeps.
We've illustrated a great amount of shape change with this person's eyeteeth, implying that a great amount of tooth structure will need to be trimmed away when making the dental crowns for these teeth. When a large amount of trimming is required, sometimes a tooth's nerve is compromised and root canal treatment is required as a part of the tooth's treatment. Only the dentist performing this work will be able to make this determination.
Up above we mentioned how some of the teeth we see in the before picture might be deciduous teeth (baby teeth). If this is the case then these teeth would not be expected to suitably support a dental crown. If this is the case then the baby teeth would need to be extracted and then these spaces filled in using either dental bridges or else dental implants. From a standpoint of our makeover illustration however, this point is academic. Dental crowns, dental bridges, and dental implants would all be expected to provide a similar cosmetic result.
